A practical on-site care model
Transportation, mobility, and scheduling can make off-site psychiatric appointments difficult for residents and staff. Mobile care removes that separate trip and allows the visit to happen in a familiar environment.
The provider can review the resident's concerns while facility information and support staff are accessible when appropriate. This helps connect the clinical conversation with the resident's actual day-to-day routine.
Services available to residents
Radiant provides psychiatric evaluations, medication management, and follow-up psychiatric care for older adults and other eligible residents. Each plan is based on the individual's history, symptoms, goals, and support needs.
Visits may address mood, anxiety, sleep, psychotic, behavioral, or adjustment-related concerns within the scope of psychiatric care.
- Initial psychiatric evaluation
- Review and monitoring of psychiatric medications
- Follow-up visits to assess progress and change
- Communication with involved family and care professionals
Working with facility staff and families
Facility staff can support the visit by preparing an accurate medication list, relevant records, and a concise timeline of concerns. Family members may also share important history or treatment goals when they are involved in the resident's care.
After the visit, clear communication helps everyone understand the psychiatric plan and what should be monitored before follow-up.
Starting a facility referral
Facility administrators, staff members, families, and caregivers can contact Radiant to discuss a referral. The office will review the resident's location, care needs, and the information needed to move forward.
Coverage varies by insurance plan, so insurance verification is recommended before services are scheduled.
